A NEW unit to support individuals in the next steps of their recovery after they are discharged from hospital has opened in Worcestershire.
The Step Down Unit in Worcester, close to the Worcestershire Royal Hospital, aims to support people who are being discharged from an acute hospital but need a period of assessment to understand what their future care and support requirements are.
This may be long term care in a care home or care in their own home. Previously, individuals would be accommodated in different locations, such as care homes, where they would receive care and support but often became more dependent.
During their stay at the Step Down Unit each person will have an individual solution developed for them.
Coun Adrian Hardman, cabinet member for Adult Social Care, said: “The Step Down Unit will increase efficiencies, improve outcomes for patients and reduce the complexity of packages of care needed to support them.
The unit has 30 beds and opened at the start of December and will build up to full capacity.
